You must complete this form and the
embedded sexual harassment training prior to starting your internship.
Internship Work Agreement Form.

You must also read, sign, and bring this form to me prior to staring the internship Harmless Hold Agreement
You must make 4 reports during the semester. The information on the length and content of these is at Reports.
This is the form the agency will complete after you have finished your internship Evaluation Form
